Checkout not working after update template

I'm using v7.0 and the checkout form will not work if I set a custom template. It looks like the form doesn't work at all, it doesn't post and the Start Date, Issue Number and No payment needed fields are all visible.

    Hi @bodhiC.
    You're including jQuery in your cached template. Take that out (it's not needed anyway because FoxyCart includes its own copy) and you'll likely be fine. If it's still erroring you can install Firebug in Firefox and check the javascript console for errors. If there are javascript errors, all bets are off on the rest of the checkout working.
